Required Registration Documents

The following documents are required for each registering student and must be uploaded and/or presented by the parent/legal guardian to the Registrar at the registration appointment. 

Upload/Bring the following documents:

  1. Proof of Residency: TWO FORMS REQUIRED (must be current documentation)
    > Parents who are homeowners (
    choose 1): property tax bill, new purchase and sales agreement, government housing letter. 
    > Parents who are renting (
    choose 1):  Signed active lease agreement or rent receipt with your name and address and property owner’s name and phone number. 

  2. Both homeowners and renters (choose 1):  Current Utility Bill: gas, electric, cable, oil, water (cell phone bill is not acceptable.)  Please be advised we cannot accept licenses, vehicle tax bills, or USPS change of address forms. 
    Please note: If you live with another family member or person, that individual will need to complete the Verification of Residency letter (Residency Affidavit) and come to Central Administration to have the signature notarized.  In addition, they will need to bring one form from each of the above listed as proof of residency requirements to prove they are Portsmouth residents.

  3. Proof of Birth: Birth Certificate, Passport, or military ID for each child you are registering

  4. Complete Physical - dated within one year

  5. Immunization Records for each child -  with month, day, and year of shots for diphtheria, tetanus, pertussis (DTP); polio; varicella (chickenpox) or proof of disease; rubella, measles, and mumps (MMR); Hepatitis B vaccine; proof of hearing screening; proof of vision screening; and proof of lead screening test; OR a certificate from a licensed physician stating that your child is not subject to immunization for medical reasons.  

  6. Parent/Guardian Photo ID     

  7. Report Card/Transcript - If applicable, please upload/bring your child’s most recent Report Card and/or Transcript when enrolling in the new school. 

  8. Request for Records - Please download this form and bring the completed form with you to your registration appointment. 

  9. Home Language Survey - To be completed at your registration appointment. 

  10. PSD 1:1 Sign-Off Chromebook Policy - This will authorize our IT department to prepare and administer your child's Chromebook device.

  11. Preschool/Early Childhood - For information about Portsmouth Preschool and Early Childhood Education Program, please contact Lauren Buress; 401-683-1650 x. 5038

  12. Any Additional Documentation (if applicable): Special Education Records (Individual Education Plans (IEP), and 504 Plans) Court Documentations (legal documentation stating custody or guardianship)

All Registration appointments take place at Portsmouth's Central Administration Office located at 29 Middle Road, Portsmouth, RI.   An appointment is necessary to complete your registration. Please note, it can take up to two school days for your child to begin school after registration.  Thank you in advance for your cooperation.
